The Username Input Dialog screen

At the top of the screen, the header "Username" is displayed.

Below is a text field for entering the username.

At the bottom of the screen there are two buttons:

• Cancel — closes the dialog without saving the username.
• OK — confirms the input; the entered username is saved and the screen closes.
